You can eat all the calcium you want. Without vitamin D, most of it passes straight through. The interesting question is not how much you take in, but how much you actually absorb.

Vitamin D controls how much calcium your body can absorb. Without enough of it, you take up only about 10 to 15% of the calcium in your diet. With adequate vitamin D, that rises to roughly 30 to 40%.* That makes vitamin D the limiting factor in the relationship: the amount of calcium you eat matters far less if the mechanism that absorbs it is not working.

This is the statistic that reframes the whole topic. According to the NIH Office of Dietary Supplements, vitamin D is required for calcium to be absorbed in the gut. Without it, the process barely functions.

Vitamin D researcher Michael Holick has put it about as plainly as it can be put: you can swim in calcium and it will do you no good without adequate vitamin D.

So the common framing, take more calcium for your bones, skips a step. If absorption is the bottleneck, adding more calcium to the top of a system that is only passing 10 to 15% of it does not fix much.

HOW IT ACTUALLY WORKS

Three stages, and each depends on the one before it.

Most supplement marketing focuses on stage one, because that is the easiest thing to sell more of. Stages two and three are where the biology actually happens.

Calcium is the material. Vitamin D is what lets it through the door. K2 is what tells it where to go.

DO YOU EVEN NEED A CALCIUM SUPPLEMENT?

Worth asking honestly, because the answer is often no.

Most people get a meaningful share of their calcium from food. Dairy, leafy greens, tinned fish with bones, fortified products. The NIH also notes that adults absorb only around 500 to 600 mg of calcium at once, so very large single doses are inefficient regardless.

Calcium supplements also carry practical downsides. The same NIH fact sheet notes they can cause gas, bloating, and constipation in some people, particularly calcium carbonate. And review-level evidence has been inconsistent on whether calcium supplementation meaningfully changes fracture risk in healthy adults.

None of that argues against calcium as a nutrient. It argues against assuming a supplement is the answer when absorption is the actual constraint.

  4. Talk to your provider before adding calcium if you have a history of kidney stones or take medications that interact with it.

Getting more calcium into a system that cannot absorb it is a strange way to solve the problem. Fix the absorption first.
